From the moment you contact Pediatric Plastic Surgery Institute, you become aware of the breadth of experience of the physicians and staff. Questions you may have regarding insurance, office hours, expectations are provided during the initial phone contact. At the time of your initial contact, please have the following information available:
- Name of Patient
- Date of Birth
- Phone Number
- Name of Insurance
- ID Number
- Group Number
- Insured’s Name (i.e. who holds the insurance)
- Insured’s Date of Birth
At the time of your first appointment, please bring a copy of your insurance card and driver’s license.

We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. Please extend us this courtesy so that other patients may be seen.
All co-payments and deductibles are due at the time of service. This includes payment for any surgical procedures, which is usually taken care of in advance.
We accept cash, check, and Visa, MasterCard and Discover credit cards.
We will bill your services directly to your insurance provider if you supply an updated proof of insurance. Please note that the contract you have with your insurance carrier is between you and your insurance provider; we are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 45 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us. As a courtesy, we will also bill your secondary insurance provider directly. Insurance CoPays and Deductibles will be collected at your first visit.
We accept patients on all Medicaid plans. Your managed care medicaid plan may not cover these services.
